Paul Robinson headed Birmingham level at St Andrews after Matt Phillips had put QPR ahead.
Phillips rifled in a left-footed shot on 17 minutes but the home side hit back seven minutes later – and the defensive shortcomings which have plagued Rangers this season were evident yet again.
Demarai Gray sent a free-kick into the box and veteran defender Robinson was able to head past keeper Rob Green.
And Gray almost put the Blues ahead when he fired narrowly wide after being set up by Stephen Gleeson a couple of minutes before the break.
Sandro is making his first start of the season, partnering Karl Henry in midfield. James Perch was also recalled to the Rangers starting line-up.
