Nahki Wells got both goals as QPR clinched a fourth consecutive league victory.
Having earlier put Rangers ahead, Wells pounced on a terrible mistake by keeper Bartosz Białkowski just a couple of minutes after Shaun Hutchinson had hauled Millwall level.
It was Wells’ fourth goal in two matches and his sixth of the season – and left the R’s fourth in the Championship table, just a point behind leaders Leeds United.
Wells opened the scoring 11 minutes into the second half after Jordan Hugill and Ilias Chair had combined nicely to lead to Rangers counter-attack.
Todd Kane’s shot was blocked and the loose ball fell to Wells, who fired into the bottom corner of the net.
Rangers, without a clean sheet this season, were pegged back with 19 minutes remaining.
They were unable to clear a corner and, after scramble in the box, former Fulham man Shaun Hutchinson poked in the equaliser.
But QPR were gifted another goal when Białkowski made a total mess of dealing with Kane’s ball towards the box and his mis-kick left Wells with a simple finish.
