Relegation-threatened Middlesex could only pick up one point on the opening day at Taunton.
Craig Kieswetter, back from injury, hit 69 off 80 balls, while Tom Abell made 50 and James Hildreth was 70 not out when the hosts reached 266-5 at stumps.
A Middlesex win would ensure they stay in Division One next season and send Lancashire down.
A draw would also be good enough if they earn maximum bowling and batting bonus points.
